# Exotoxins, Endotoxin and How Damage Is Actually Done

Some damage comes from secreted proteins with precise targets inside host cells, and some from a membrane component released when Gram-negative cells break up. The two differ in specificity, in potency and in whether heat destroys them.

## Lesson: Two ways germs poison you

Germs damage you in two opposite ways. Exotoxins are proteins that living cells release, each aimed at one target inside your cells. Endotoxin is the lipid A part of the outer membrane of Gram negative cells, harmless while anchored and dangerous when millions of copies spill out as cells break apart.

**Example.** Two named diseases make this concrete. Diphtheria toxin travels from a throat infection and halts protein synthesis in heart and nerve cells far away. Cholera toxin locks gut signaling switched on, which floods the gut with water. Each exotoxin maps to exactly one disabled process.

Treatment can briefly make things worse with Gram negative germs. An antibiotic that bursts cells floods the blood with endotoxin, and the immune overreaction can spike fever or drop blood pressure before you improve. Heat tells the two apart: boiling unravels protein exotoxins, while the lipid endotoxin survives, so removing it needs far harsher treatment. Antitoxin antibodies can help against an exotoxin disease even while antibiotics handle the germs, since antibodies neutralise the toxin directly.

**Tip.** To sort a toxin, ask three questions: does a living cell release it, does it hit one precise target, and does heat destroy it. Yes to all three means exotoxin. A heat tough general poison spilled from broken Gram negative membranes means endotoxin.
