Ventilation and Perfusion Matching, and the Control of Breathing
Gas exchange only works where air and blood arrive in the right ratio, and local vessel and airway responses steer both towards the regions that need them. Breathing rate is set mainly by carbon dioxide sensed as a change in pH, not by oxygen.

Gases move down their pressure gradients: oxygen is high in the sacs and low in arriving blood, so it diffuses in, while carbon dioxide runs the opposite way. But both flows must reach the same spot. A region with air but no blood is dead space, and a region with blood but no air is a shunt, and either mismatch leaves blood gases worse.
Hold your breath and the urge to gasp comes from rising carbon dioxide, not falling oxygen. Central receptors in the brainstem track carbon dioxide through changes in pH and drive most of the response. Peripheral sensors in the neck and chest watch oxygen instead, but they stay quiet while oxygen is normal and only shout when it falls far, which is why oxygen is the backup signal.
When carbon dioxide climbs, breathing quickens and deepens to blow it off. Yet extra breathing alone cannot fully fix a shunt, because blood passing unaired sacs never picks up oxygen no matter how hard the healthy regions pant. Mismatch must be fixed at its own region, not by overworking the rest.
For any breathing complaint, first ask which side is missing: air without blood, or blood without air.
