Ventilation in Insects, Fish and Mammals
Ventilation keeps the concentration difference high by replacing the medium at the exchange surface. Tracheal pumping, countercurrent flow in gills and pressure changes in the thorax are three answers to one demand.

Ventilation never changes the surface itself. Its real job is refreshing the air or water at the exchange surface so the concentration difference between outside and blood stays steep. Without that refresh the oxygen by the surface runs out, carbon dioxide builds up, and diffusion slows to a crawl, however huge the area.
Watch your own chest to see pressure at work. When your chest expands, the space inside grows and the pressure inside drops below outside, so outside air rushes in from high pressure to low. When your chest relaxes back down, the space shrinks, pressure inside rises above outside, and air flows back out.
Fish show the neatest version of gradient keeping. Water flows one way across the gill plates while blood inside flows the opposite way, a setup called countercurrent flow. The water beside each bit of blood always carries slightly more oxygen, so oxygen keeps diffusing in along the whole plate instead of balancing out halfway.
Insects pump air through tracheal tubes with muscles and air sacs, fish push water one way over gills, and mammals pump air with chest pressure changes. Three setups, one shared goal: keep the gradient high. If an answer says ventilation widens the surface, it has mistaken the gradient job for an area job.
Tracheal pumps, countercurrent gills and chest pressure all serve one goal: a steep gradient.
