The Autonomic Nervous System and Its Two Outflows

A separate motor system runs the organs without conscious control, through two outflows that leave the cord at different levels and usually oppose each other. Because each outflow uses its own transmitters and receptors, most drugs that affect heart rate, pupils or gut motility act here.

Your organs get two rival autopilots. The sympathetic outflow leaves the middle cord, thoracic to upper lumbar, and relays in chain ganglia hugging the spine. The parasympathetic outflow leaves through cranial nerves plus the sacral cord, and relays in terminal ganglia tucked beside the organs. Most organs hear from both, pulling opposite ways.

Preganglionic fibers in both systems speak acetylcholine to nicotinic receptors in the ganglia. Past the ganglia they differ: sympathetic fibers whisper norepinephrine to alpha and beta adrenergic receptors, while parasympathetic fibers continue with acetylcholine. Phenylephrine, an alpha-1 mimic, shows the logic: it dilates the pupil and raises blood pressure.

Block a receptor and you mute one autopilot's voice. A beta blocker interrupts adrenergic drive, so heart rate and pressure fall. Block alpha-1 and the pupil cannot widen. Drugs that mimic the transmitters do the reverse: sympathomimetics copy norepinephrine's orders.

Each adjustment runs as a reflex loop: sensors such as the aorta's baroreceptors report stretch, cranial nerves carry the news in, the cord or brainstem integrates, and a two-step efferent path answers, preganglionic fiber to ganglion, then postganglionic fiber to the organ. Some loops finish locally in a short reflex; most travel the long road through the cord.

Two opposed outflows use their own transmitters and receptors, so each drug mutes or mimics exactly one voice.
